Requesting Holy Baptism (Submitting a Baptism Application Form)

The sacrament of baptism is available to anyone who wishes to become part of the body of Christ, which has become known as the Church.

To request baptism, simply complete a Baptism Application Form, using either the MSWord version or the PDF version, then email the attached form to the Rector at rector@saintnicks.com.

The rector or the parish administrator will then get back with you to discuss scheduling opportunities for the baptism ceremony and the necessary preparation.
